About The Service

Independent Living Services (ILS), Adult social Care, and Cardiff and Vale UHB work in partnership with vulnerable adults with the aims to keep older people socially connected, independent and able to remain safe and secure in their own homes for as long as possible. Health and social care professionals work collaboratively with people, using a person-centred care approach which supports people to make informed decisions about “what matters” to them.

This is an exciting opportunity to join a new and developing service, to work with a wide range of professionals and to make a real difference to people and families. The post will be located in University Hospital Wales, and potentially other sites within the city.

The role will support the Integrated Discharge Hub, in order to ensure effective deployment of domiciliary care workers and to support the safe and timely discharge of patients from hospital to home. Working with individuals, their carers, therapists and community support to enable people to achieve their optimal level of independence. You will use outcome focused support planning to promote the independence, health and wellbeing of citizens, while at the same time giving them more choice and control over the support they receive. This post is part of a Regional Integration Fund initiative.

What We Are Looking For From You

You will need to be a motivated individual with a passion for promoting independence through a strength-based approach as well as an excellent communicator with the ability to listen, act and inform. You will able thrive in a dynamic multi disciplinary team. As well as being confident working on your own.

The ability to speak other languages, in particular Welsh, and Community Languages would be an advantage.
