Blaenau Gwent IAA Team Principles:

We are looking for a strong, dynamic and enthusiastic leader, with excellent knowledge of Preventative practice and experience of working in Adult Social Care. Experience of supporting change management would be desirable.

The Senior Practitioner will be able to use their role to support a cultural change within existing staff through coaching, mentoring, direct supervision developing the skills of existing staff across a number of partners to improve the interface and support individuals to achieve their wellbeing outcomes

This should result in an increase in people meeting their own needs, signposted to relevant services or supported to access integrated preventative services that reduce hospital admissions, long term care admissions or increased packages of care.

Thorough pre-employment screening is undertaken during the recruitment process. The postholder will be required to complete an application for a Disclosure of Criminal Records.

This post requires a registration with Social Care Wales under the category of Social Care Managers / Social Workers and registration will need to be in place prior to commencement in the role.

Blaenau Gwent County Borough Council
Company Information

Blaenau Gwent is defined physically by the beautifully desolate mountain tops which divide and watch over its vibrant, bustling valleys. Blaenau Gwent is a place of heritage and dramatic change, with a sense of its past but its eyes firmly on the future.

Although the towns give the county borough a busy, urban feel, Blaenau Gwent is actually a largely rural area. Forty five per cent of the land area is undeveloped, and the greater part of this is defined as open countryside. Wherever you are, beautiful countryside is never far away, and as often as not it will be providing the views from where you’re standing.
