Purpose Of The Post:

  • Ensure that the people of Gwynedd are at the heart of everything we do.
  • Help individuals to live their lives as they wish to live.
  • Take on the full care role in the home.
  • Collaborate with other agencies, for the benefit of the individuals, including attending reviews, meetings with health, consulting with specialists.
  • Administration of medicines

Responsibility For Functions:

  • Ensure appropriate use of users' property e.g., medicine and cash.
  • Make appropriate use of specialist equipment to assist the individual to live his/her life as they wish to live. This includes reporting on deficiencies and following practice protocols and specialist training as required.

Main Duties:

Support The Individual Who Use The Services

Role - full charge of home role if appropriate

  • Take responsibility for the residential care home and the responsible person role.
  • Lead staff with their work duties.
  • Take decisions in the absence of managers, e.g., receive admissions back from hospital, lead in an emergency, respond to enquiries.
  • Lead and respond to an emergency situation involving users and the building
  • Identify changes in the health and well-being of individuals and respond quickly and appropriately to them.
  • Be progressive and willing to make informed decisions independently, while also applying professional opinion and knowing when to attract support from the wider team and other professions.

Care Worker Duties:

  • Excel in building relationships with people to understand exactly what would make a difference to them in their lives and empower the voice of the individual at every opportunity.
  • Be innovative and creative when giving care in a way that fully includes people, that continuously strives to include people in even the simplest of activities and enable or re-able people to take control of their own lives.
  • Be a key worker for individuals attending meetings and contribute orally and in writing
  • Maintain and support the procedure for administering and distributing medicines in accordance with the service policy.
  • Promoting the rights of the individual and encourage them to take positive and informed risks.
