About Us:

Vale Community Resource Team are an Integrated service run by Vale of Glamorgan Council and Cardiff &The Vale UHB. We provide short term support and/or therapy to people living in their own home when they are first discharged from hospital or to prevent a hospital admission. Our aim is to maximise a person’s independence.

About The Role:

An exciting opportunity has arisen to join our integrated multidisciplinary team. As one of our Reablement Support Workers you will support people in their own homes with daily living tasks such as personal care and meal preparation.

Main Duties And Responsibilities: 

The job description refers to the principal duties and responsibilities of the Post. It does not necessarily list in detail all the tasks required to carry out these duties and responsibilities.

  • To provide the highest possible quality of care to the people we support in accordance with their Personal Plan and to actively promote their independence.
  • To work in accordance with a planned rota for delivery of a flexible service for the whole of the Vale of Glamorgan which is available between 0700 and 2230, 7 days per week.
  • To provide support to people as stated in their Personal Plan, tasks on mobile phone app and any other relevant documents as directed by the Reablement Coordinators.
  • To deal with complex and emergency situations as and when required seeking support and guidance from The Reablement Co-ordinators and other members of the multi-disciplinary team.
  • To make use of relevant communication systems (Mobile phones, email and MSTeams) to keep up to date and to pass on information.
  • To lone work in the community, travelling to visit the people we support in their own homes
  • To build rapport with a wide range of people including the people we support and their families on a continual basis as VCRS is a short-term service
  • To support people with daily living tasks including personal care, meal preparation, light domestic duties, laundry and shopping.
  • To support with continence care and/or catheter care including changing day/night bags.

The Vale of Glamorgan Council is the governing body for the Vale of Glamorgan, one of the Principal Areas of Wales.
