Overall Job Purpose:
To work as an integral part of the Community Mental Health Support Services Team (CMHSS). To provide low, medium or high mental health support in accordance with their needs to individuals referred to the service in order to maintain or achieve their personal outcome and support recovery.
This is achieved by identifying and implementing a bespoke outcomes focused care and support plans for individuals. Emphasis is placed on enabling service users with enduring and serious mental health issues to live their lives as independently as possible within their home environment and their communities, and be able to acquire and / or to maintain secure accommodation and to manage their day to day living and accommodation effectively.
An individual, on initial referral will be allocated by the relevant Coordinator who will discuss, agree and plan outcomes with each individual based on care and support needs. The Support Worker will focus on supporting the individual to achieve the agreed outcomes in the care and support plan and review within the agreed timescales.
